Physical Therapist

Job Description:

Our client has a full-time Physical Therapist opening, serving patients throughout Clay County, Florida. This field-based home health role gives you the autonomy to manage your schedule while delivering meaningful, one-on-one care in patients’ homes.

You’ll play a key role in helping individuals regain mobility, improve function, and restore independence—while being supported by a collaborative interdisciplinary care team.

What You’ll Do

• Conduct comprehensive in-home physical therapy evaluations
• Develop and implement individualized treatment plans
• Deliver skilled interventions to improve strength, balance, mobility, and independence
• Educate patients and caregivers on exercise programs, fall prevention, and safety
• Communicate patient progress and condition changes with physicians and care team members
• Complete accurate and timely documentation per agency and regulatory standards
• Participate in interdisciplinary case conferences as needed

What We’re Looking For

• Active Physical Therapist (PT) license in Florida
• 2+ years of physical therapy experience preferred (home health experience a plus; strong clinicians welcome to apply)
• Strong clinical judgment, communication, and time management skills
• Ability to work independently in a field-based setting
• Valid driver’s license and reliable transportation

Schedule & Work Environment

• Full-time position
• Field-based: patient homes throughout Clay County
